Project Description
Brisas Paliativo (Brisas Palliative) provides specialized at home health care services for patients with chronic diseases. Our focus is to offer continuity in the treatment process, as well as physical, psychosocial and spiritual support. The main objective is to improve the quality of life of both the patient and his family. This service is meant for patients with chronic illnesses who leave the hospital, but still do not qualify for hospice.
It has been shown that patients receiving palliative care in the home improve their quality of life and reduce readmissions, which consequently reduces the costs of medical care. Clinical studies have shown the positive impact of this type of care in patients with chronic diseases
WHO IS ELIGIBLE FOR THESE BENEFITS?
The patient is eligible for palliative care when:
A doctor (if they have one) and the medical director of hospice care certify that the patient is terminally ill and that he/she probably has less than six months to live.
The patient signs an agreement choosing hospice care instead of the ordinary benefits for his terminal illness.
